Potentates’ Activity @ Colorado National Speedway May 19^th.
          Display of Units Mid-afternoon    and
        Intermission performance at 7:00pm.

Please indicate your intentions for the above parades by circling yes or no and return this form to Joel P. Roche’ Sr. Director of Units. If yes, your unit will be required to complete a unit request for insurance coverage and approved by the Potentate or the Director of Units.

Units attending any other "non-Shrine" parades must register on their own with Parade Committees. A Unit Request must be submitted for any of these parades and approved by the Potentate or the Director of Units and be on file in the Business Office.
